What should a client intake form ask?

A useful client intake form collects contact details, goals, scope, budget, timing, files, stakeholders, approvals, and communication preferences without asking for information the client has already provided. The right questions depend on the service and project risk.

Keep the form proportionate to the project

A short consultation may only need goals, timing, and contact details. A larger design or marketing project may also need files, stakeholders, approval rules, and budget constraints. Remove any question that does not change how you scope, price, schedule, or deliver the work.
